Doc-owned group launches Harlem medical home

Manhattan's Physician Group, New York, has partnered with insurance provider EmblemHealth to create a patient-centered medical home in the city's Harlem neighborhood.
EmblemHealth will offer point-of-care services to allow physicians at the multispecialty practice to coordinate educational and community resources in addition to medical services.
Patients will have electronic medical records to help streamline care among physicians, labs, pharmacists and hospitals, according to a Manhattan's Physician Group news release. The technology also will allow patients to have their prescriptions filled electronically and communicate with doctors via email.
The goal of the program is to reduce readmissions and improve health outcomes, according to the release.
Six of Manhattan's Physician Group’s seven practices have received Level 3 recognition—the highest possible—as a patient-centered medical home from the National Committee for Quality Assurance.
The new space in Harlem will replace an office on the Upper West Side. The 18,000-square-foot facility will house offices for primary care, obstetrics and gynecology, gastroenterology, infectious disease and nutrition specialties for adults and children.
